Chronic Disease Self-Management Program
A chronic disease self-management program - "I'm In Control" - is available at the Pike County Health Department, Home Health & Hospice. Residents with chronic diseases such as heart or lung disease, pain, cancer, etc. are candidates for this program.
“I’m In Control” is aimed at teaching self-management skills designed to assist those with chronic illness to gain overall control and improve overall well-being.
